Thank you for your question. The patient expired value sets received some discussion in terms of harmonization during the 2014 annual update. However, the governance group decided the expired value sets could not be completely harmonized among all MU2 eCQMs within that timeframe and therefore, the harmonization was partially completed.

"Hospital Measures - Expired 2.16.840.1.113883.3.666.5.1146" has been replaced in the following eCQMs:
in AMI-2, it is replaced by "Patient Expired 2.16.840.1.113883.3.117.1.7.1.309"
in ED-3, it is replaced by "Expired 2.16.840.1.113883.3.666.5.730"

In order to create a harmonized and fully implementable "expired" value set, we would like to hear from vendor community, what codes you use. The QDM attribute requires SNOMED-CT codes, but in reality there may be ICD-9 codes or HL7 codes.

    • Semantic drift caused by limited values in value set

The Hospital Measures - Expired value set (2.16.840.1.113883.3.666.5.1146) had only 2 values in it.
R99 Ill-defined and unknown cause of mortality ICD10
      798.9 Unattended death ICD9

The problem is that not very few deaths in a hospital or ER are unattended (ICD9) nor are they all ill-define and unknown cause of mortality (ICD10)

This value set forces one of those two options, but if a patient is in a code situation neither fits. Could we just have a generic 'Expired' code?
