      Thank you for your question regarding CMS506v7, Safe Use of Opioids - Concurrent Prescribing. This measure uses QDM datatype, "Medication, Discharge", which is described in the Quality Data Mode, Version 5.6 (https://ecqi.healthit.gov/sites/default/files/QDM-v5.6-508.pdf). As documented on pg. 40-41, “[d]ata elements that meet criteria using this datatype should document that the medications indicated by the QDM category and its corresponding value set should be taken by or given to the patient after being discharged from an inpatient encounter.” The measure does not intend to capture medications that are not on the discharge list such as the Percocet included in your example. We encourage you to work with your EHR vendor to ensure medications that are canceled or discontinued are not inappropriately captured by the measure.

      The logic should select only the most current or most recently completed discharge reconciliation, rather than ALL reconciliations, because the latest prescribed discharge reconciliation is the one sent to the pharmacy for the patient to continue at home.]

      However, both Oxycodone & Percocet were included in the numerator 
      The report takes into consideration ANY AND ALL Medications Reconciliation - this patient had 3 discharge medication reconciliation completed.
