      Thank you for your question regarding CMS0002 Preventive Care and Screening: Screening for Depression and Follow-Up Plan. eCQMs are designed to use clinical data captured in structured fields within the EHR. Clinically equivalent concepts documented in the medical record may be mapped to the codes included in a measure’s value sets to populate measure elements. However, we are not able to provide guidance on how specific codes can be mapped; we recommend you consult with your EHR vendor and clinical partners. If mapping is conducted, you should maintain documentation in case of a CMS audit.

      Additional question regarding this issue and follow up questions relating to the responses provided in issue CQM-8262

      Question 3. The response to issue CQM-8262 noted that information provided is applicable to CMS2  version 14 of the specification. Will the feedback provided in that issue be applicable to CMS2 version 15 as well? 

      Question 4. Relating feedback provided in issue CQM-8262 and for this issue, documentation by providers that are not an action of an order or an action of a referral, but are documentation of a plan, is that documentation (examples noted above and in issue CQM-8262) enough to satisfy an audit or will additional documentation be needed within the record to support what is mapped to the SNOMED codes in the approved valuesets?
