Question 1: Is mapping the documentation (list below) to the SNOMED listed with it acceptable to meet the measure as a follow up plan or referral?
Question 2: Are there any that should not be associated (mapped)?
Question 3: If there are any that should not be associated, is there a recommendation of a more appropriate option?
- “Continue Current Treatment”- SNOMED 410231007 from value set 2.16.840.1.113883.3.526.3.1568
- “Referral to BH”- SNOMED 81294000 from value set 2.16.840.1.113883.3.526.3.1568
- “Referral to Counseling”- SNOMED 306226009 from value set 2.16.840.1.113883.3.526.3.1571
- “Referral to Spiritual Care”- SNOMED 711281004 from value set 2.16.840.1.113883.3.526.3.1571
- “Prescribe Medications”- SNOMED 410234004 from value set 2.16.840.1.113883.3.526.3.1568
- “Patient Education”- SNOMED 410224008 from value set 2.16.840.1.113883.3.526.3.1568
- “Other” (This option comes with a free text field that can be left blank or the provider can add free text documentation to the Follow Up Plan) SNOMED 410231007 from value set 2.16.840.1.113883.3.526.3.1568
Additional question regarding this issue and follow up questions relating to the responses provided in issue CQM-8262
Question 3. The response to issue CQM-8262 noted that information provided is applicable to CMS2 version 14 of the specification. Will the feedback provided in that issue be applicable to CMS2 version 15 as well?
Question 4. Relating feedback provided in issue CQM-8262 and for this issue, documentation by providers that are not an action of an order or an action of a referral, but are documentation of a plan, is that documentation (examples noted above and in issue CQM-8262) enough to satisfy an audit or will additional documentation be needed within the record to support what is mapped to the SNOMED codes in the approved valuesets?
Thank you,
