You should also note that your first bulleted option (No treatment needed based on clinical judgment) will not meet measure compliance. The score on the validated depression screening tool determines whether a follow-up action is required. The measure allows for providers to use their clinical judgement to determine the extent of the intervention provided (e.g., exercise therapy, SNOMED 229065009 if stress-related). Therefore, you may want to use caution with options that could negatively impact measure compliance.

      We acknowledge and have reviewed the eCQM definition for  Follow Up Intervention for Positive Adult/Adolescent Depression Screening includes the following: ["Medication, Order": "Adult Depression Medications"  union ["Intervention, Order": "Referral for Adult Depression"] union ["Intervention, Performed": "Follow Up for Adult Depression"].

      Our providers thoroughly evaluate symptoms, severity, and patient mental status to establish a diagnosis and manage positive depression screenings. To ensure accurate capture of these critical follow-up plans, our organization has approved and implemented a standardized workflow: during annual wellness or regular Evaluation and Management (E/M) office visits, providers are directed to attest to their follow-up plan interventions related to positive depression screenings via a dedicated drop-down menu within their progress notes that includes the following:

      • No treatment needed based on clinical judgment
      • Patient currently treated with medication and/or therapy
      • Referral to mental health provider
      • Appropriate medication prescribed
      • Follow visit for additional treatment scheduled
      • Patient refusal of treatment/referral
      • Plan per provider

      We are requesting guidance on appropriate assignment of SNOMED 410231007 Mental health screening management (procedure) _ _which falls under child OID 2.16.840.1.113883.6.96 and parent OID 2.16.840.1.113883.3.526.3.1568 (Follow Up for Adult Depression) to map these provider follow up planning interventions for eCQM reporting purposes.
