Clinically equivalent services may be mapped to the codes used in CMS117v14 to satisfy the numerator requirements. If the vaccine includes multiple antigens, it can be mapped to multiple value sets. For example, you may map a code for each of the individual antigens included in the vaxelis vaccine (e.g. one code for DTaP, IPV, HiB, and Hep B). If mapping is done, you should maintain documentation in case of a CMS audit.

      Can you please provide guidance on proper mapping of the Dtap-Ipv-Hib-Hepb vaccine for Vaxelis specifically? We see this maps to CVX 146 but that requires a 4-dose scheduled, but on the manufacturer website they specify that this is a 3-dose vaccine. This is causing our physicians/APP to fail the measure because they are administering 3-doses for Vaxelis.
