Hello,
I'm writing to get clarification in reference to resolution/comment posted on CQM-6304 and CQM-8213, as the responses differ. In order for us to clarify, could you please confirm the outcome for the scenarios below with multiple providers within a shared EMR?
This is a patient-based measure CMS2 in a shared EMR, and the measure specification mentions the most recent screening to be considered.
- Multiple screenings by different providers throughout the year.
Provider John has an encounter and negative screening in March.
Provider Mary has an encounter and positive screening in December with no follow-up.
As December is the most recent screening for the patient, do both providers not get numerator credit? If not, which providers get numerator credit?

2. Given there is a requirement for a screening to occur during or within 14 days of the qualified encounter, in the scenario below where:
Provider Sam has an encounter in January and no screening.
Provider John has an encounter and negative screening in March.
Provider Mary has an encounter in December and no screening.
Does Provider Mary also get Numerator credit? Provider Sam had an encounter in January which is before screening, would he also get numerator credit? Or they don’t get credit as their encounters are not within 14 days from the screening?

Provider John has an encounter and Positive screening on 1st of March.
Provider Trace has an encounter on 5th March and no screening.
Provider Kris has an encounter and does the follow-up on 15th March.
Provider Mary has an encounter in December and no screening.
Do all these providers get numerator credit? If not all, which providers get numerator credit?
