GMCS: overlap of loinc code between risk screening and nutrition assessment

      Thank you for your inquiry regarding CMS986/GMCS (Global Malnutrition Composite Score). For Reporting Year 2024, use of one of the NRS codes is recommended if the screening uses the NRS. The joint LOINC code (84291-4) should be used if a different screening tool is used. Please note significant updates to terminology in the value sets for Reporting Year 2025 provide better, generic options for alternate valid screening and assessments for this measure.

      Regarding measure CMS-986 GMCS, I have a question about LOINC code 84291-4, which is used in both the Malnutrition Risk Screening and Nutrition Assessment value sets.

      In CQM-6211, you say “If you are utilizing a screening tool other than the NRS-2002, we recommend use of the general code (LOINC, 84291-4)”. 

      In CQM-6129, you say “The LOINC code, ‘84291-4 Nutrition and dietetics Risk assessment and screening note’ is recommended for Measure Observation 1 “Malnutrition Risk Screening” and Measure Observation 2 “Nutrition Assessment” when a registered dietitian (RD) or registered dietitian nutritionist (RDN) performs both the "Malnutrition Risk Screening" and "Nutrition Assessment".”

      Given this guidance, what should an organization do if nurses (not dietitians) do the risk screening with a tool other than the NRS-2000?  It seems our only option is to map one of the NRS-2002 specific LOINC codes for a different risk screening tool because we don’t want the nursing risk screening to also satisfy the dietitian nutrition assessment.  Is this allowed or do you have other guidance?
