      A patient prescribed or continuing a prescribed antithrombotic at hospital discharge will meet the intent of the measure, as long as there is appropriate documentation (i.e. discharge medication list).

      Numerator Header

      Inpatient hospitalizations for patients prescribed or continuing to take antithrombotic therapy at hospital discharge.

      Numerator Logic:

      TJC."Ischemic Stroke Encounter" IschemicStrokeEncounter

        with "Antithrombotic Therapy at Discharge" DischargeAntithrombotic

      This is a follow-up question to CQM-6418 and would like to confirm the following example:
      Based on the response, are we correct in understanding that in the example, where a patient is prescribed Aspirin 81 mg on 3/5/22 and the patient's IP stay started on 4/25/2022, with a discharge date of 4/26/22, and the prescribed Aspirin 81 mg on 3/5/22 had a day supply that exceeded the discharge date, this patient could be included in the numerator?
