      We limit to tests specifically for “streptococcus pyogenes” because it is specific to “Group A strep.” The LOINC code 546-2 for Beta Hemolytic Strep Culture is not specific to “streptococcus pyogenes", therefore, it is not included. Anything that is not specific to “streptococcus pyogenes” may be testing for some other type of strep which does not meet the intent of this measure.

      We currently have a number of patients failing the measure, because our clinicians have ordered Beta Hemolytic Strep Culture. They feel this test is more helpful as it will indicate presence of any type of Strep. It seems inappropriate to fail the measure for doing appropriate testing just because the result could include more than the minimum required.

      During  recent investigation  on CMS-146, we noted LOINC code 546-2 for Beta Hemolytic Strep Culture is not included as a qualifying Strep A test. We found issue CQM-2582 which had previously inquired about the same. The resolution indicated the issue had been resolved in the 2018 update; however, there is no rationale explaining why this code is not acceptable, since a Beta Hemolytic Strep Culture is going to tell you whether or not  patient has Strep A.
