This measure is an episode-based measure meaning that the measure is to be reported for every eligible encounter during the measurement period. The measure logic defines eligible encounters as those included in the Encounter to Document Medications value set (2.16.840.1.113883.3.600.1.1834). We refer you to the measure specifications located on the eCQI Resource Center for the measure specifications and logic at https://ecqi.healthit.gov/sites/default/files/ecqm/measures/CMS68v11.html

      This value set includes CPT, SNOMED, and HCPCS Level II codes for the eligible encounters included in the measure. CPT code 99211 is included in this value set and is therefore considered an eligible encounter. You can review the complete list of codes for this value set on the Value Set Authority Center (VSAC) at https://vsac.nlm.nih.gov/. You can review codes for specific value sets by entering the value set number into the Query field on the "Search Value Sets" tab or you can download a list of all value sets and codes used in CMS68v11 from the "Download" Tab by clicking on the 2022 Reporting/Performance Period of eCQM & Hybrid Measure Value Sets option and opening the May 2021 release of the eCQM & Hybrid Measure Value Sets, with the publication date of May 06, 2021.

      Is there a list of CPT codes that count towards this measure? The is a list provided for the MIPS CQM version of this measure, but I have been unable to find anything for the eCQM version.

      An EMR vendor is counting Nurse Visits (99211) in the denominator of this measure, even though the visit doesn't require an Eligible Clinician. This CPT code isn't included as an Eligible encounter for the MIPS CQM version.
