There is very little guidance on how to handle (or exclude) patients that expire in either EH or EP measures.
Currently, I've found only two definitions that reference deceased. They are 0712 and 1354.
Each measure should explicitly state how to handle an expired patient. Are they included in the initial population? Are they excluded from the denominator?
Also, this logic should follow and match the logic used elsewhere on cms.gov. For example, in PQRS and the GPRO, a deceased patient is considered Not Qualified for Sample for the same NQF that might be referenced in CQM.
