Patients admitted for elective carotid intervention

      The eMeasure development team and the steward (the Joint Commission) have realized that using the reason attribute in the Encounter QDM to represent elective procedures is not ideal, not just because the reason attribute does not reflect the true intent, but also as the vendor raised, it is difficult to standardize the data source for this attribute in EHRs. In the coming MU2 annual release (4/2013), the team decided to refine the Inpatient Encounter value set and eliminate the codes pertaining to elective admissions. As a result, the remaining codes reflect the intent of non-elective admissions. In addition, the logic phrase

      OR: "Occurrence A of Encounter, Performed: Inpatient Encounter (reason: 'Carotid Intervention')"

      is also removed from the Denominator Exclusions. In doing so, the measure will NOT include the patients with elective procedures in Initial Patient Population and therefore, there is no need to define the exclusion criteria (cited above) anymore.

      The logic phrase defining elective carotid intervention has been removed in the MU2 annual update. Instead, the encounter value set has been refined to remove all elective admission codes. Patients with elective procedures will not enter IPP at the first place and therefore, QRDA documents should not be concerned with elective procedures anymore.

      1) We are struggling to capture the concept of 'Patients admitted for elective carotid intervention' in our EHR. The statement implies that the reason of the inpatient admission has be a Carotid Intervention, but the eMeasure doesn't define it as a principal diagnosis or principal procedure of the encounter. How are EHRs expected to capture 'reason for admission'?

      2) We are also struggling to place the concept in the QRDA Cat I file as none of the sample patients provided so far by Cypress, through QRDA Cat I or HTML, have Carotid Intervention mentioned and the HL7 QRDA Cat I guide does not have a concept related to 'reason for admission'. Since it is not a principal procedure or principal diagnosis, where do you put 'Carotid Intervention' in the QRDA Cat I?
