2017 Q4 Reporting Period eCQM Value Set Addendum cannot be adopted for Q4 only

      UPDATED GUIDANCE: CMS appreciates your continued feedback on the 4th Quarter 2017 value set addendum and the challenges both EHR vendors and health systems are facing to comply with implementing this update. CMS understands that integrating the 4th Quarter 2017 value set addendum into EHR products may be infeasible in certain EHRs. While CMS prefers that the 4th Quarter 2017 value set addendum be integrated and applied for the 4th Quarter 2017 reporting period only, for eCQM reporting requirements for hospitals under the Hospital Inpatient Quality Reporting and EHR Incentive Programs, submitting the 4th Quarter 2017 without the addendum will not result in a file rejection but may affect the measure performance. CMS continues to encourage hospitals to select one quarter from 1st Quarter – 3rd Quarter 2017 to meet the reporting requirements if integrating the 4th Quarter 2017 value set addendum into EHR products is infeasible. In other words, reporting one quarter from 1st Quarter – 3rd Quarter 2017 would not necessitate integrating the 4th Quarter 2017 value set addendum.

      CMS considers applying the 4th Quarter 2017 value set addendum to the entire 2017 calendar year as inappropriate as it could skew reporting results for the first 3 quarters of the year. Similarly, CMS considers delaying the implementation of the 4th Quarter 2017 value set addendum to calendar year 2018 as inappropriate because a separate 2018 value set addendum has been issued and should be applied to that year.

      Because of the feedback we have received, CMS is considering how best to approach the desire by some stakeholders to make use of updated codes in a more timely fashion while balancing the operational aspects of integrating such updates, and welcomes continued dialog on this topic through this ticket. Since we were responding to several stakeholder requests to provide 4th Quarter updates, CMS would like to hear your suggestions and thoughts on whether and how to integrate potential future mid-year value set changes into products and EHR systems which will be both helpful and feasible to all stakeholders in future years.

    • Cannot implement CMS September 2017 eCQM addendum as published.

      We are reviewing the 9/1/17 CMS email titled "CMS Announces Updates to eCQM Value Sets for Q4 2017 Reporting, and 2018 Reporting and Performance Periods" and are concerned that the email indicates that the new value sets are to be uesd for Q4 2017 and 2018. If a new value set is to be adopted for 2017 it must be retroactive for the entire year. Adopting a new value set for Q4 2017 only is not feasible.

      Please advise on if it's preferable to adopt the new value sets for the entire 2017 year or defer value set updates until 2018.
