      The intent of this measure is to examine actual medication administration, not medication orders, and the logic matches the intent. The measure criteria ask whether medication has been given to the patient or not, and, if not, is there a documented reason in the EHR. Since the measure looks for the evidence of medication given or not given, the measure does not use medication order data. A medication order does not always trigger the medication administered.

      In the logic for the denominator exclusions, it includes the following for medication administered not done: OR: "Medication, Administered not done: Medical Reason" for "Unfractionated Heparin RxNorm Value Set" OR: "Medication, Administered not done: Medical Reason" for "Low Molecular Weight Heparin RxNorm Value Set� OR: "Medication, Administered not done: Medical Reason" for "Factor Xa Inhibitor RxNorm Value Set� OR: "Medication, Administered not done: Medical Reason" for "Warfarin RxNorm Value Set" The value sets for these appear to be more appropriate for Medication Order not done.� I do not believe that a nurse should be making the judgment� for these medical reasons.� The measure does not have any logic for medication order not done.� Is this an error in the logic or is the expectation that the person administering the medication make this clinical judgment?
