      The visits do not need to be with the same provider. In your example, a patient could be counted in the IP and denominator for both providers if the patient had 1 visit with the Doc 1 and the second visit with Doc 2, as long as the information about the visits is available in both providers’ EHRs.

      CMS CQM 77v2 and CMS 77v3 Initial Population and Denominator
      Should Occurrence A and Occurrence B visits both have occurred with the same provider in order to meet the Initial Population and Denominator requirements?
      OR
      Could Occurrence A visit be by Provider Doe1 and Occurrence B visit be by Provider Doe2 ? And since both Occurrence A & B visits are at least 90 days’ or more apart during the measurement period, both providers would meet denominator even if each provider only had 1 visit with that patient.
