      In general, the data from the record populates regardless of whether the provider seeing the patient has taken the required action or not. However, each measure is constructed to relate the activity in question as appropriate to the intent of the measure. If the measure seeks to get all providers to perform smoking cessation, it should look for smoking cessation activities regardless of the provider. If the measure seeks to get the patient to have recommended screening, it should look for any instance of that screening in the appropriate interval. If the measure looks to see that a specific physician followed up on a finding or performed a reassessment, the measure logic should link the two encounters with the physician.

      Please refer to issue CQM-1413, which provides further instruction on how to address data elements recorded by or actions taken by other providers in regards to measure performance. Note that this principle does not apply if it violates the measure logic.

      We have a multi-specialty practice sharing an EHR. If the primary care physician documents some of the information, will the specialist be able to report on that as well - ex. Orthopedic specialists may not typically do smoking cessation intervention.
      However, the PCP would document that information on the same patient that the Ortho specialist would see, so could the Ortho specialist report that cessation was performed if it was performed by the PCP ??
