      The concept of occurrencing is to identify a given instance/event and point back to that same instance/event in subsequent blocks of logic. Once the instance is identified (eg, the Clinical Staging Procedure with a Distant Metastasis Status M0 that occurred on 3/5, and the patients record showed an Office Visit that occurred on 5/3), the timing constraints don’t need to be repeated in the logic, as all other instances of Clinical Staging Procedure point back to that same instance/event identified (eg, the instance that occurred on 3/5).
      For additional information regarding the concept of occurrencing as used in eCQM’s, please see section 3.3 of the Logic Guidance & Release Notes document titled, “Specific Occurrences”, posted in the eCQM Library.

      With regards to the IPP for this measure-
      If the measurement period is Jan 1- Dec 31, 2014...
      Does this statement mean the FIRST time the patient was diagnosed with active breast cancer, 5 years or less any time before Dec 31, 2014
      Ex. So if patient had an active dx at encounter visit= March 31, 2014 only
      We will look back 5 years or less from March 31, 2014?
