For the numerator condition, if a medication is active prior to occurrence A and still active after occurrence A, you may count the days it is active after occurrence A as well as any medications that start after occurrence A. In your example,

1.Medication is active before 90 days for N numbers of day prior of occurrence A and after occurrence it is active for M number of days.
so what will be the cumulative days count
2.Medication will start after the occurrence A and we will count the 84-114 days after it (P number of days).

The number of cumulative days would be M + P.

There are two scenario which are possible.
1.Medication is active before 90 days for n numbers of day prior of occurrence A and after occurrence it is active for m number of days.

2.Medication will start after the occurrence A and we will count the 84-114 days after it.
Do we also count the days before 90 days of Occurrence A of medication dispense in cumulative days count for medication active so totals day will be #(N+M)
      or
Only the days after Occurrence A of medication dispense(IPSD) so total count will be of #M cumulative days.
