Need guidance on how to calculate med duration on active meds with no end dates

Currently unable to calculate medication duration for active meds with no defined end dates (since med is active)
How are we to calculate medication duration for active meds with no end dates?
EX: CMS179v1
Cypress test data contains a patient with the following meds:
Warfarin (1)
start: May 10, 2011
end: UNK
Warfarin (2)
start: March 1, 2012
end: UNK
Warfarin Total Duration: 391 days
Questions:

1) What reference end date is used to handle individual medication duration when the medication is active with no defined end date?
- start of reporting period?
- end of reporting period?
- date of record import?
2) How was the "391 days" summary duration produced by Cypress calculated?
We've referred to the eCQM logic doc and JIRA, but have not been able to identify appropriate guidance. Any direction on where this is documented or how we should calculate this duration would be very much appreciated.
