Multiple entries of diagnostic study results will be created in this use case. For example, if you want to say, “Macular exam: severe retinopathy AND Macular exam: macular edema”, you would have two instances of the Diagnostic Study Result. Each would have the same observation/code, but they would differ in their observation/value (and in their observation/value/@sdtc:valueSet).

CMS167/NQF0088 – Diabetic Retinopathy: Documentation of Presence or Absence of Macular Edema and Level of Severity of Retinopathy – requires MULTIPLE results for a single Diagnostic Study.

The QRDA-I spec allows only a single result. This contradiction needs to be resolved by either allowing multiple results within a single entry, or providing a recipe for providing multiple diagnostic study result entries with the same ID and timestamp.
