The Hospital Quality Reporting (HQR) QRDA Category Release 2 Supplementary Implementation Guide, Version: 2.2 (4/21/2014) has an updated specification for Race, Ethnicity. On page 6 of the report, it has the following language:
“In the event where race is unknown or patient declined to provide, a nullFlavor is permitted.”
“In the event where ethnicity is unknown or patient declined to provide, a nullFlavor is permitted.”
If race and ethnicity for the patients are null and if according to your EHR system, this needs to be interpreted as unknown, then you could report as <raceCode nullFlavor=”UNK”/>. If the patient declined to provide therefore it was null in your EHR system, then set the nullFlavor code to be “ASKU”.

If the fields for payer and zip code are null for patients, nullFlavor of “NI” could be used.

For gender, as the value set contains code “U” (Unknown), if the field is null for patient, you could report using the code “U”.
