Request identifier attribute for Provider, Characteristic

      QDM-80 (now closed) requested details about individual providers and the issue was discussed in the QDM User Group previously.  The new request is to add an identifier attribute to enable measures expressions specifically addressing individual providers. Such is not possible in QDM versions through QDM 5.4.
