Consider Re-Specifying Family History

      The QDM currently represents family history using Diagnosis, Family History. Aside from having a strange name (coupled to Diagnosis), it also does not support common aspects of family history (such as identifying the relationship to the patient).

      FHIR's Family History allows for each family member to be identified by:

      • name
      • relationship to the patient
      • birth date and/or age
      • deceased indicator and/or deceased date
      • conditions, including onsets and outcomes

      C-CDA R2's Family History Organizer allows for each family member to be identified by:

      • relationship to patient
      • gender
      • birth date and/or age
      • deceased indicator and/or deceased date
      • problems, including effective times

      The QDM user group should consider breaking Family History from its ties to diagnosis, as well as re-specifying its representation in QDM.

      QDM UG Accepted Solution (May 2015)

      Rename Diagnosis, Family History to Family History. Family History will be in its own QDM category and support the following attributes:

      • relationship: the relationship of the family member to the patient
      • onset age: the estimated or actual age in years of the family member when the diagnosis/problem began
      • recorded datetime: the date/time the family history entry was recorded

      The value set applied to Family History will indicate the diagnosis/problem the family member has (or had).

      Note: This solution removes the following attributes from the existing Diagnosis, Family History data type: ordinality, status, patient preference, provider preference, negation rationale, start datetime, stop datetime.
