Yesterday I have created test cases for this measure CMS125v14 - Breast Cancer Screening https://madie.cms.gov/measures/66b13bdfee2fb93c15793366/edit/details/
It was 100% Passing and 100% Coverage.

Today 8/12/2025 I opened the same measure - It has 5% Passing and 3% Coverage.

It appears that Encounters don't have Codes. And also there are no Codes available in a dropdown:
