The 3 measures in question are CMS117v6, CMS136v7, and CMS147v7. This is using Cypress version 3.2.1.
For CMS117v6, Cypress is expecting Ross Bailey to be in the population. For CMS136v7, Cypress is expecting Michelle McCormick to be part of the population. And for CMS147v7. Cypress is expecting Grace Barrett to be part of the population. In all 3 of these cases, 2 QRDA1 files were present in the test deck for the patient in question. And in all 3 of the cases, there wasn't enough information included in the QRDA1s to place the patients in the initial patient population.
For Ross Bailey, one QRDA1 had immunization entries. The other QRDA1 only had a payer code. There were no encounters in either of the QRDA1s. For Michelle McCormick, one of the QRDA1s had all the information needed for the patient to enter the numerator (including an encounter that would place the patient in the numerator). However, there was never an encounter to place the patient in the initial patient population - the second QRDA1 only reported a payer code. For Grace Barrett, both of the QRDA1s only reported a payer code.
