CMS22v5 test deck has a patient with a negated procedure that doesn't have a reason

      I'm using Cypress 3.1.0. I was testing using the test deck for CMS22v5 and I noticed that a patient had a procedure that didn't have a reason in the QRDA. I don't think this is intentional as the patient information lists a reason.

      I have attached the file from the test deck. I do not know if there are more patients like this in the test deck, this was just the first one I noticed.
