We cannot locate CMS guidance for how to submit 2014 CQM clinical data from an EHR for ambulatory providers.

For MU2, electronic submission is required. We were told on Cypress calls to expect guidance from CMS in November of 2013 about how this would work. Hospital guidance was provided by CMS but there is no guidance for ambulatory providers using their EHR to submit – that I have been able to find.

I am aware from the Cypress discussions that CMS is expecting an export consisting of a QRDA 1 – one patient/all measures – but I have seen no official documentation of that. I hesitate to change our product until I see the official spec. Perhaps this is optional and only QRDA 3 is required for submission.

Also, I don't know where you would electronically submit. Is there a URL? Is it via an ftp? It is probably very similar to how PQRS works, but not being particularly informed about that, I need the documentation.

There was some discussion in the last Cypress call about the identifiers used in the submitted files for hospitals. Where is the comparable documentation about the required ID's for ambulatory submissions?
