Value sets differ for Meaningful Use and Core Measure abstraction regarding "reason for no VTE prophylaxis" .

    • Thank you for your question. While the eCQM medical reasons value set specifies generic reasons for not providing VTE prophylaxis, it would be acceptable to map more explicit reasons to one of the codes in the eCQM value set.

      It has come to our attention that the 2014 Meaningful Use requirements for “reasons for no VTE prophylaxis” contradict with the specification guidelines for Core Measure abstraction. Reason for no pharmacological VTE prophylaxis must be excplicitly documented, ie; severe anemia, active gi bleed in order to meet the core measure indicator. However there is a list of reasons that must be used in the EMR and cannot be added to or substituted for Meaningful use submission. This table includes:

      Procedure contraindicated
      Treatment not indicated
      Drug treatment not indicated
      Absent response to treatment (
      Drug resistance)
      Complication of medical care
      Medical contraindication
      Treatment not tolerated
      Not needed
      Not indicated

      When abstracting VTE core measures how should we address documentation by the physician in response to this question?
