Thank you for your inquiry regarding the Advanced Illness and Frailty exclusion criteria for the 2027 Performance Period. These codes were removed because they do not align with the intent of the frailty exclusion. The frailty criteria are intended to identify individuals with indicators of persistent frailty, including long-term reliance on assistive devices.
Clinically equivalent services may be mapped to the codes used in a measure's value sets to satisfy the numerator or denominator requirements. If mapping is done, you should maintain documentation in case of a CMS audit.

    • This affects our ability to provide a 'Walker' option to our customers for capturing this type of Frailty device.

The Frailty Device Value Set (2.16.840.1.113883.3.464.1003.118.12.1300) no longer contains SNOMED codes for capturing Walker. The 4 options listed below were removed for 2027. This presents a problem for us because we need to offer a Walker option to our EC's to capture for Frailty Device. Please advise on how we should capture the use of a Walker.

1255320005 Wheeled walker (physical object) SNOMEDCT
1256013004 Gait rehabilitation electronic wheeled walker (physical object) SNOMEDCT
1256014005 Basic non-foldable wheeled walker (physical object) SNOMEDCT
1256015006 Basic foldable wheeled walker (physical object) SNOMEDCT
