Thank you for your question regarding CMS506v9 Safe Use of Opioids - Concurrent Prescribing. The measure does not currently include intra-facility transfers because it has not been tested in those settings. We will explore this through testing in a future Annual Update cycle.

Our healthcare system has a Swing Bed, ARU and LTACH which are merely located on different floors of the same hospital building. When patients are transferred to these internal spaces, the "discharging provider" may be covering the same patient as they move to the other internal location. Providers may not want to discontinue the pain regimen meds that the patient is taking while on one unit as they move to another "hospital" within the same building. The provider may not discontinue home meds on the patients discharge list until they are discharging that patient from the hospital all together, as this interferes with workflow and efficiency. Why are all Intra facility transfers not excluded?

Referencing the given Response on previous ticket CQM-7640 SUO Discharge Disposition Exclusions:

"Please note, patients who are admitted to an inpatient encounter and subsequently transferred to a skilled nursing level of service within the same facility (swing beds) should not be included in the measure. eCQMs are not able to differentiate patients based upon level of service. We encourage you to work with your EHR vendor to remove these patients from the measure for purposes of reporting. We will look into ways to address this in a future version of the measure."

If our hospital has an internal Rehab Unit, LTACH, SNF and Swing Bed, would we exclude all of those internal transfers? Or does this only apply to swing beds?

Our EMR has the following Disch Dispositions. Would we exclude the highlighted?
      1. ARU

      2. ARU w/ Planned Readmission (used for internal transfers) (AND Deprecated ARU w/ planned readmission)

      3. SNF

      4. Swing Bed

      5. Swing Bed w/ planned readmission (used for internal transfers) (AND Deprecated Swing Bed w/ planned readmission)

      6. LTACH

      7. LTACH w/planned readmission (used for internal transfers)

      8. AMA

      9. Expired
