As specified in the guidance section of the measure, "If the glycemic status assessment (HbA1c or GMI) is in the medical record, the test can be used to determine numerator compliance." The measure requires documentation of an HbA1c or GMI assessment, identified by the "HbA1c Laboratory Test" (2.16.840.1.113883.3.464.1003.198.12.1013) value set or the "Glucose Management Indicator" LOINC code (97506-0), along with the test result and date. If the glycemic status assessment is documented in the medical record and represented using the appropriate terminology included in the measure specification, it may be used for measure evaluation. The measure guidance also states that glycemic status assessments must be reported as a percentage (%).   We do not provide prescriptive guidance on mapping the value sets. Equivalent services can be mapped to the measure's value sets.
