For example, if a specific VTE prophylaxis was indicated but not administered, the documentation would capture the medical reason along with the specific medication to support the change in treatment guidelines. This approach is used to replicate the intent of the legacy measures to identify deviations from national practice standards.

      Medication, Order not done: Medical Reason not done for <medication value set > and Medication, Administration not done: Medical Reason not done for <medication value set>. We see this in several measures and are interpreting it to mean that a code from the Medical Reason value set must be submitted in conjunction with the medication value. Is our interpretation correct? In a typical clinical workflow, clinicians normally document why a medication is not ordered or not given (i.e. bleeding risk), but they typically do not document what medication would have been given (RxNorm, medication value set level) in conjunction with the reason.
