Is it sufficient to support the value set for 'Moderate or severe LVSD' or is it required to support 'Diagnosis, Active: Left ventricular systolic dysfunction' value set also and include a provision to identify the severity of the diagnosis

      eMeasures often include multiple OR'd clauses which provide for multiple alternative approaches when identifying numerator or denominator patient populations. Any single clinical record is expected to contain only a single alternative. The reason for multiple options is to be flexible for vendors to facilitate a smoother transition towards newer diagnostic codes.

      When designing an EMR implementation (or the EMR itself), the designer need only consider the identified alternatives and determine how to best implement _at least one_ of the alternatives - they are not required to implement more than one. So in your specific case, it would be acceptable to only implement one approach, such as submitting pre-coordinated concepts that combine severity plus LVD, if that works best for you.
