      Thank you for the follow up question about CMS136v15 (2027 performance period). It is correct that CMS136v15 also uses the QDM datatype “Medication, Active” in addition to “Medication, Order” when determining the Initial Population and Denominator. However, the same principle applies: only structured medication entries that meet the QDM datatype definition and are coded to the measure’s value sets are considered. Patient reported medications do not count unless they are documented in the EHR via QDM datatype "Medication, Active" with appropriate codes found in the measure logic.

      A follow up question in response to: CQM-8701 CMS136- Patient Reported Medication - Office of the National Coordinator for Health Information Technology  The logic also includes the QDM datatype of "Medication, Active" for the IPP and Denominator.  Just to confirm a patient reported medication should  not count towards this data element?  For example, patient states they are taking an ADHD medication prescribed by another provider, that patient SHOULD qualify even though they are previously on an ADHD medication, just not prescribed by our clinic?
