Anesthesia type, start and end date-times are merged into one data element

      Thank you for your question. After discussion with the steward, the following change was made to the logic during the April 2014 measure annual update: “<= 1 day(s) starts after end of ("Procedure, Performed: General or Neuraxial Anesthesia" <= 1 day(s) ends after start of "Occurrence A of Encounter, Performed: Inpatient Encounter") Other than this change, the steward would like to leave the logic as it is. With this change both the nested phrase and the first part are looking at the end of the anesthesia procedure. Once a patient meets the criteria of having a surgical episode which ends within 1 day after the inpatient encounter starts, the next step would be to look at whether the VTE prophylaxis did or did not occur within 1 day after the end of the anesthesia for the same surgical episode.

    • Unable to accurately capture anesthesia start and end timing

      The criterion “<= 1 day(s) starts after end of ("Procedure, Performed: General or Neuraxial Anesthesia" <= 1 day(s) starts after start of "Occurrence A of Encounter, Performed: Inpatient Encounter") expects one data element which combines information about the anesthesia type and its start and end time. However, in our medical record this information might be captured by several separate data elements. While there would be a date-time associated with the capture of the anesthesia type, it would be the date-time the anesthesia type was recorded not the intended date-time. Is it possible to have an alternate component of the specification where these criteria are separate? Other CQMs make use of separately recorded date-times that are needed for calculations, e.g., Baseline State in NQF 0437.
