Specifically, we would like to confirm whether providing a patient with an ambulatory referral to a smoking cessation program (Referral to Tobacco Use Cessation Counseling Program), or utilizing an AHEC Smoking Cessation pre-referral (Referral to Tobacco Use Cessation Counseling Program), would satisfy the numerator criteria for this measure.

Additionally, we would appreciate clarification on whether placing a referral to a tobacco use cessation counseling program is considered equivalent to counseling being provided for the purposes of meeting the numerator if clinicians provide counseling for cessation alongside the referral, or if separate documentation of counseling delivered is required.
