Thank you for your inquiry regarding CMS871v4, Hospital Harm - Severe Hyperglycemia.

Measure Observation 1 (associated with the denominator) is defined as the count of eligible hospital days of inpatient hospitalization, which match the initial population/denominator criteria and did not meet the denominator exclusion criteria.

Measure Observation 2 (associated with the numerator) is the count of eligible hospital days of inpatient hospitalization that meet the definition of a hyperglycemic event and did not meet the numerator exclusion criteria.

Inpatient hospitalizations shorter than 24 hours are effectively excluded from the measure. Though a hospitalization less than 24 hours may technically meet the denominator criteria, it cannot meet the numerator criteria, because a hospitalization can only be included in the numerator population if it has a hyperglycemic event day in the first 10 days of the hospitalization, excluding the first 24 hours of the hospitalization and the last period before discharge from the hospital if less than 24 hours.

Since the measure does not count any hyperglycemic events that occur in the first 24 hours, day 1 of a hospitalization is not considered an eligible hospital day for the measure observations. Because the measure calculation relies on data from two observation measures, hospitalizations lasting less than 24 hours are excluded from the final calculation and patients admitted less than 24 hours are not included in the total.

"The measure does not count any hyperglycemic events that occur in the first 24 hours, day 1 is not considered an eligible hospital day for the Measure Observations. Eligible days range from day 2 up to day 10."

The above statement is part of numerator comment.  How is the National and State percentage calculated for HH-Hyperglycemia? Can you define what the numerator and denominator or total patient count is? Specifically, I would like to know if patients that have been admitted for less than a day are included in the total?
