      Thank you for your inquiry regarding CMS1218v2, Hospital Harm - Postoperative Respiratory Failure. This measure uses the "Albumin lab test" value set (OID: 2.16.840.1.113762.1.4.1248.221) to identify laboratory tests measuring albumin in serum or plasma but does not require a specific method such as BCG (bromocresol green). For purposes of reporting this electronic clinical quality measure (eCQM), one of two LOINC codes found within the “Albumin lab test” value set must be documented. However, equivalent services may be mapped to either of the codes in the value set, if the result is captured and recorded as an albumin mass or moles per volume result and reported in g/dL or umol/L. We are unable to provide specific guidance related to the mapping of codes and further recommend you consult with your electronic health vendor (EHR) vendor and clinical parters. If mapping is conducted, you should maintain documentation in case of a CMS audit.

      For CMS1218v2, we noted Albumin lab test (2.16.840.1.113762.1.4.1248.221) include the following generic LOINCs
      LOINC 1751-7 Albumin [Mass/volume] in Serum or Plasma
      LOINC 54347-0 Albumin [Moles/volume] in Serum or Plasma
       
      This more specific LOINC, as well as other more specific LOINCs, is not included in the value set: 
      LOINC 61151-7 Albumin [Mass/volume] in Serum or Plasma by Bromocresol green (BCG) dye binding method
       
      Was excluding a more specific albumin testing method intentional for this Value Set? We are concerned that the specification's limited albumin LOINC listing may lead to under-representation of albumin results.
