We have an OP-1 patient that is showing a mismatch between our EHR vendor and our quality reporting vendor. Our EHR vendor shows that the patient is in the denominator. According to our review, this is what we see as well. Our quality reporting (QR) vendor shows that this patient is excluded. When we questioned this, our QR vendor stated that the exclusion was being pulled from a 2024 file that we had loaded into their system with the code of F11.20. Our EHR vendor confirmed that this code is in a 2024 encounter, but does not exist in the 2025 files and is not in the 2025 problem list. 

Should we be using a diagnosis to exclude this patient that is not active during the reporting period?
