      “Not tolerated” appears to be an EHR-specific selection, and not part of the actual screening tools referenced. However, the measure has a “Medical Reason” denominator exception, which includes SNOMED CT code 407563006 (Treatment not tolerated (situation)). If the “Not tolerated” the button is selected in your EHR, AND your reporting vendor has the button mapped accordingly, AND a depression screen was not completed during the measurement period, then the patient will be considered exempt from requiring a depression screen.

      "Medical Reason(s) 
      Documentation of medical reason for not screening patient for depression (e.g., cognitive, functional, or motivational limitations that may impact accuracy of results; patient is in an urgent or emergent situation where time is of the essence and to delay treatment would jeopardize the patient's health status)”

      1) Will the "Not Tolerated" selection on the PHQ2/PHQ9 exclude a patient from the depression screening measure(s) if selected?

       

      2) If the "not tolerated" button does not exclude the patient from the quality measure, what action should be taken if a patient has dementia or other condition that hinders their ability to complete the PHQ2/PHQ9 accurately?
