Thank you for your inquiry regarding CMS125v14 (2026 Performance Period). The eCQMs are designed to use clinical data from structured fields in the EHR, as opposed to billing/claims data. For the numerator criteria, the measure identifies at least one documented mammogram in structured EHR fields, ensuring that it was performed within the required time frame and documented via QDM datatype "Diagnostic Study, Performed" using a code from the "Mammography" value set (2.16.840.1.113883.3.464.1003.108.12.1018). The mammogram(s) can be performed any time between October 1 two years prior to the measurement period to the end of the measurement period. Having a result from the mammogram is not a requirement to meeting numerator compliance, but the measure specification requires documentation as expressed in the logic below.

Please review the measure specification for further details about measure requirements: https://ecqi.healthit.gov/sites/default/files/ecqm/measures/CMS125-v14.0.000-QDM.html. If you have questions about reading the measure specification or understanding data requirements, you may refer to the "Guide for Reading eCQMs" for additional guidance. If you have questions regarding implementing the measure, you may refer to the "Implementation Checklist for eCQM Annual Update" for additional guidance. These resources can be found in the eCQI Resource Center: https://ecqi.healthit.gov/ep-ec?qt-tabs_ep=ecqm-resources&global_measure_group=eCQMs. If you have questions regarding CMS quality program reporting requirements, please review the QPP website or contact the QPP Helpdesk at QPP@cms.hhs.gov.

Does CMS allow for the use of claims data to satisfy the numerator criteria for the breast cancer screening measure? For example, if we receive claims data from payers that include the mammogram procedure code that was completed and the date it was performed, are we able to use this information to satisfy the numerator criteria for the measure, or are we required to obtain the report in order to use this information to satisfy the numerator?
