      For STK-3 (Anticoagulation Therapy for Atrial Fibrillation/Flutter), if a patient had atrial fibrillation documented in 2016 but is admitted again in 2025 with no documentation of atrial fibrillation during the new encounter, should the STK-3 measure for the 2025 admission use the 2016 atrial fibrillation diagnosis, or does the measure rely only on atrial fibrillation documented during the current encounter?
