      Thank you for your inquiry regarding CMS826v3: Hospital Harm - Pressure Injury. The eCQM assesses inpatient hospitalizations for patients who develop a new stage 2, 3, 4, unstageable, or deep tissue pressure injury. Inpatient hospitalizations with a stage 2, 3, 4, unstageable, or deep tissue pressure injury present on admission (POA) as indicated by a POA indicator of "Y" or "W" or identified by a skin exam within the first 24 hours (for stage 2, 3, 4, or unstageable pressure injuries) or 72 hours (for deep tissue pressure injuries) of the inpatient hospitalization meet denominator exclusion criteria and are not included in the measure. Because these inpatient hospitalizations are excluded from the measure's denominator, they will not be included in the measure's numerator population, even if the patient develops a pressure injury at a new anatomical site or if the initial pressure injury deteriorates to a higher stage during the inpatient hospitalization.

    • Clarification for the numerator value

      The current exclusion description states that patients with a Stage 2, 3, 4, Unstageable pressure injury or DTPI diagnosis POA.

      Does that mean if this patient develops a new pressure injury elsewhere on their body they still remain an exclusion? Is this patient now eligible to be placed in the numerator based on the new hospital harm?
