As specified in the guidance section of the header, "If the glycemic status assessment (HbA1c or GMI) is in the medical record, the test can be used to determine numerator compliance." This measure requires documentation of an HbA1c or GMI lab test, identified by the "HbA1c Laboratory Test" (2.16.840.1.113883.3.464.1003.198.12.1013) value set or the "Glucose Management Indicator" LOINC code (97506-0), along with the test result and date. A patient reported test may be counted toward HbA1c or GMI testing as long as the results are sent to a laboratory or the provider’s office for analysis. Documentation of the criteria noted above must be recorded in the structured fields of the EHR for measure evaluation.

      We received a question from a practice about whether they can report CMS122v14 when the HbA1c value is patient-reported and then entered into a discrete HbA1c field in their EHR. The practice does not have an electronic interface to outside labs and does not have access to official lab reports/results.

      We noted that the CQM Specifications state: "Do not include HbA1c or GMI levels reported by the patient." We're seeking confirmation on whether this same restriction applies under the eCQM specifications for CMS122v14.
