      Thank you for your inquiry regarding CMS826v2 and CMS826v3, Hospital Harm - Pressure Injury. This eCQM is an episode-based measure. An episode is defined as each inpatient hospitalization or encounter that ends during the measurement period. As an episode-based measure, this eCQM assesses each inpatient hospitalization separately.

      If a patient develops a pressure injury during each of their inpatient hospitalizations during the measurement period, then each inpatient hospitalization would meet the measure's numerator criteria. However, please note that only one harm (new qualifying pressure injury) is counted per hospitalization. So, if a patient develops multiple pressure injuries during a single inpatient hospitalization, that inpatient hospitalization would only be included once in the measure's numerator.

      Suppose a patient has several admissions during a reporting quarter and sustained a hospital-acquired pressure injury at each visit.  Should this medical record be combined into one folder for data submission to CMS?  If combined, does each PI count or only the first one?
