      Thank you for your inquiry regarding CMS996v6: Appropriate Treatment for ST-Segment Elevation Myocardial Infarction (STEMI) Patients in the Emergency Department (ED).

      Angina pectoris with documented spasm; aneurysm of the heart; ventricular aneurysm due to, and following, acute myocardial infarction; Takotsubo cardiomyopathy; Takotsubo syndrome; long-term current use of anticoagulants; discharge disposition of discharge to home for hospice care; discharge disposition of discharge to a health care facility for hospice care; and hospice care minimum data set assessment are not value sets, but direct reference codes. A direct reference code (DRC) is an individual code referenced directly in the electronic clinical quality measure logic.

      The list of direct reference codes for the 2026 reporting period can be found on the VSAC “Downloadable Resources” page [https://vsac.nlm.nih.gov/download/ecqm]. On that page, locate the section labeled “2026 Reporting/Performance Period of eCQM & Hybrid Measure Value Sets,” which corresponds to the May 08, 2025 release. Within this section, there is a table listing all downloadable resources. The link to download the list of direct reference codes is located in the last row of the table: “Direct Reference Codes Specified within eCQM HQMF files Published May 08, 2025.” Please note that you may need to create a free account to view this page and download the file.

Unable to locate diagnosis value sets for: angina pectoris with documented spasm, aneurysm of heart, ventricular aneurysm due to and following acute myocardial infarction, takotsubo cardiomyopathy, takotsubo syndrome, and long term current use of anticoagulants. The non-diagnosis value sets unable to locate are: discharge disposition of discharge to home for hospice care, discharge disposition of discharge to health care facility for hospice care, and hospice care minimum data set assessment.
