A diagnosis can contribute to the numerator only if it is included as an encounter diagnosis; therefore, a diagnosis listed on the problem list would affect the measure only if the organization’s extraction or mapping process includes it in the encounter’s diagnosis list.

      Present on Admission (POA) status is determined solely from the POA indicator on the encounter diagnosis record (and is not inferred from the problem list). For purposes of this measure, the numerator diagnosis logic evaluates encounter diagnoses with POA coded using the “PresentOnAdmissionIsNoOrUnableToDetermine” [2.16.840.1.113762.1.4.1029.370] value set, which includes POA values of No or Unable to Determine. Final diagnoses are typically coded with present-on-admission indicators in billing/claims data. We recommend working with your EHR vendor to identify the best way to ensure POA indicators are reported with encounter diagnoses for the measure.

      Can diagnosis that are on the problem list but not on  the final diagnosis list under the hospital account be pulled into the numerator for the measure?  If yes, and  they are pulled into  the measure how is it determined if they are  they are POA= yes or no?
