within the CY2026 PFS QPP final rule, CMS125V14 and CMS130V14 numerator definitions were expanded to include the verbiage "and reviewed by a clinician"   .  These two measures allow previous year procedures to meet measure requirements, with breast cancer screenings in the 27 months prior to the end of the measurement period, and colorectal screenings up to 9 years prior to measurement period.  How does CMS intend to grandfather procedures outlined as acceptable in these two measures prior to the implementation of this requirement change which starts in 2026.  

In other words, How will CMS take into account colorectal screens and breast cancer screens completed prior to 2025 when reviewing and potentially auditing these mesaures for patients included in the measure population for measurement period 2026 and beyond?
