      This measure is designed to ensure all recommended portions of the foot exam are completed. If the single data point 100% of the time indicates all 3 portions of the foot exam were completed, then you could map it to the 3 corresponding SNOMED codes. However, if you cannot ensure that the foot exam single data point covers all 3 portions of the foot exam 100% of the time, then the single data point does not meet the intent of the measure.

    • Confused on how to capture data elements.

      The Diabetes Foot Exam measure specifies that a visual, sensory AND pulse foot exam be performed during the reporting period. We're a data warehouse, so we don't have control over source system data, and we typically see diabetic foot exams as a singular data point. We've developed this measure to discretely capture each of these data points if available, but would it acceptable to also capture one all encompassing foot exam data point and then break out into the corresponding SNOMED-CT codes upon generation of the QRDA I files?
