Thank you for your follow-up question on CMS1017v2, Hospital Harm - Falls with Injury (HH-FI) and CMS1218v2, Hospital Harm - Postoperative Respiratory Failure (HH-RF). HH-FI collects data on all encounter diagnoses, their diagnosis ranks, and their Present on Admission (POA) indicators for the purpose of collecting data for the ongoing re-evaluation of the measure's risk adjustment model. These encounter diagnoses and their ranks are not currently used for the risk adjustment of measure scores. Therefore, if an implementer does not submit a rank with a patient's encounter diagnosis, this omission will not affect the risk adjustment of the implementers' measure scores for HH-FI. HH-RF collects data on (1) all encounter procedures and their procedure ranks and (2) each patient's primary procedure during each encounter (where procedure rank = 1) for the purpose of collecting data for the ongoing re-evaluation of the measure's risk adjustment model. Similarly, these procedures and their ranks are not currently used for the risk adjustment of measure scores. Therefore, if an implementer does not submit a rank with a patient's procedure, this omission will not affect the risk adjustment of the implementers' measure scores for HH-RF.
